Saline vs. Silicone
Breast Implants come in different sizes and shapes. They can be filled with either saline or can be silicone gel. Saline and silicone breast implants can be placed below the pectoral muscle, in a sub-pectoral pocket. At Fox Plastic Surgery Center in Naples, FL; Dr. Elizabeth Fox can help you choose which option is better for you. There are advantages and benefits for both types of implants such as:
Saline Breast Implants
Possible to achieve a high round look
Smaller incision therefore, smaller scar
Less expensive than silicone
Scar is located around the areola; camouflages well
Silicone Breast Implants
Look and Feel more like natural breast tissue
Less rippling and wrinkling than saline implants
Can be placed either above or below the muscle

Implant Placement
Dr. Fox can increase the volume and fullness of your breasts by placing a saline or silicone breast implant either above or below the pectoral muscle. At her AAAA certified surgical facility in Naples, Dr. Fox mostly places breast implants beneath the muscle for the following reasons:
Below the Pectoral Muscle
Looks more like natural breast tissue
Better visualization on mammography
Slows breast drooping in the future
Helps impede breasts from sagging in the future
Less wrinkling and rippling
Above the Muscle
An implant may be placed above the muscle if you are trying to avoid the incisions of a breast lift in the immediate future
Increases chance of rippling and wrinkling
More palpable (can be felt easier than if under the muscle)
May require a breast lift in the future
Incision Sites
There are four basic incision sites used for breast augmentation. Dr. Elizabeth Fox will discuss your options with you and help you decide which is best for you.
Transumbilical or Endoscopic

An incision is made in the belly button and the surgeon tunnels through the abdominal tissue to reach the proper placement

No warranty is honored on implants when the transumbilical approach is used

Very unlikely that implant will be placed under the muscle
Axillary Approach

An incision is made in the axillary (underarm) area and the surgeon makes the pocket from the side
-
Less likely to achieve adequate cleavage, implant is placed in a more lateral (out to the sides) position

More than six weeks of discomfort after surgery, longer healing time

Tearing of muscle, too much destruction of tissue
-
You may have a visible scar when you wear a sleeveless shirt
Periareolar

An incision is made on the side of the areola (the colored part of the nipple)

May be used for saline implants or small silicone implants

Heals beautifully and scar may be barely visible

Scar blends well with natural change in skin color surrounding the nipple

Allows for placement of the implants under the muscle or under the glandular tissue

An incision is made under the crease of the breasts, in the natural fold under the breast

Revisional Surgery
Dr. Fox is an expert at revisional surgery; she often performs corrective surgery on women who have had multiple surgeries by other surgeons. Anesthesia
Dr. Elizabeth Fox believes in providing the best for her patients, therefore, she only uses IV Sedation with Propofol. Similar to the anesthesia used for a colonoscopy, IV sedation is a lighter type of sedation than general anesthesia; while the patient remains totally asleep. At Fox Plastic Surgery Center in South Florida, our all female staff of five nurses and a female anesthetist is nurturing, efficient and experienced in plastic surgery patient care. We have a AAAA certified surgery center for your comfort and safety. All of the Surgical Staff are advanced cardiac life support (ACLS) certified because Dr. Fox demands only the best for her patients.

Does not require a tube down your throat

Does not alter your pulse and blood pressure levels like general anesthesia

Typically does not cause nausea or that "sick" feeling caused by general anesthesia

Recovery
Dr. Fox and her nursing staff will go over everything with you in great detail before your surgery. We've thought of everything, so you will be prepared emotionally and physically for your surgery. You will want to be home to rest and do very little activity for the first 5-7 days. Here are a few more pointers:

Dr. Fox uses only dissolvable sutures for her patients; all sutures will be under the skin

Dissolvable sutures lead to a better result, less visible scar without stitch marks

You will go home with a surgical garment that looks like a tight tube top, it is a pressure dressing

Pain medicine is prescribed for your comfort

No strenuous exercise or heavy lifting for 4-6 weeks
